On Your First Visit

Please bring with you:

  • your health insurance cards (Medicare, Blue Cross, PPOM, AARP...)
  • a list of your current medications
  • the name and phone number of your primary care physician and pharmacy

We understand that the cost of health care is of concern to all of us. Our office accepts assignment with Medicare, Blue Cross, PPO, Cofinity, and most health insurance companies. However, for HMO and Managed Care patients, we would like to remind you that it is your responsibility to obtain a referal from your primary care physician for treatment when required.
